Bad PSU?

I think I may have a bad PSU that may be dying. It is a generic 480watt PSU with 18amps/12v, that came with my PC case which I bought about 2-3 months ago. Recently, when playing games such as CSS, suddenly the monitor would go black and the fans would slow down a bit, but the computer still turned on. So I decided to put everything at stock speeds, and it was fine. Whenever I overclock and increase the voltage by too much, the PSU (I believe) will fail and can't support it.
Look at the attachment, Speedfan/Everest shows that it has 12.74 on its 12v (where I believe the max should be 12.5). This leaves me in believing everything is my PSU's problem. Am I right?
If so, what is the best PSU that fits my rig that can be bought under $50? I don't need a really good one, but as long as its stable.

Thanks, but those PSU's seem to cost a bit more than my budget. How are these PSUs

All of them seem pretty decent to me. But what is the difference between 24-pin and 20+4-pin? Are they the same? How do I know which pin my mobo supports?

How much are you selling it for? I found out that my mobo supports 20 pins, so no 24 pin PSU for me. What is the difference between 20-pin and 20+4-pin PSU? I hope they're the same.

From what I understand a 24 pin is for when you run SLI, but I could be wrong.I have a 20 pin in my MB, but it will hold a 20pin and it seems to work just fine. I'm on it now. I want 35.00 shipped is all. I still have the box it came in
