There isn't really any benefit from having the PCI-E Freq above 100Mhz with a single card. There's normally enough bandwidth, but try raising it and see.
With scores that low the card must be going into low power mode. I'd suggest sending the psu you've just bought back and going for another one.
Sunbeam Nuuo 550W, OCZ Powerstream 520W, Seasonic 500W will all be able to power that card.

It almost looks like the memory timings on the vid card have gone wayward???? Try getting another psu and wire it up so it runs(easy 1 wire needed) and just have that dedicated to running the vid card and see if that helps? assuming you can borrow one from somebody maybe or have another one. You arent folding at the same time are you?

My thoughts exactly when I rigged up one two power supplies ago. I rigged up one dedicated to it to no avail. I am at my wits end. I think i am just going bite the bullet and get another card. Oh yeah I have also flashed the bios. Winflash reports the card as a 32m card. I dont know if thats correct and frankly after a another weekend of a machine that acts like its had a labotomy I couldnt care. Its broken me.

See if you can somehow connect the two 12V rails together and see if that helps.
