Have you made sure that the Card is fully In the AGP slot and that you have all Drivers updated. Drivers would however mak no difference in the Boot-up so make sure that the FAN and Heatsink on that card are fully seated properly and that the fan is working as i reckon temps are to blame here..Other than that your card could be old and the GPU is on its way out.

Although the fan is WORKING, dosen't mean that it isn't overheating, thus producing artifacts.

Do you have the card overclocked any?

And is it AGP or PCI?

Also if you have your Computer Overclocked please make sure you have the PCI/AGP ports locked at there Defaut speeds..As if they are not they could cause the symptoms you are seeing.

Depends, check out what I did to my videocard:

Possibly you could try this and it may work. It's done a GREAT job of allowing me to overclock my card.

LOL pwned!

Grid lines usually are a sign of getting hot. Do you have some thermal paste? I would remove the HSF, clean off the old stuff, and then apply some new.
