Like I said, I doubt you will find a case that's horizontal that you can fit a big gfx card

I do agree with the others that a Gaming PC should be a seperate rig...

I was running my UD3R with a Q9650 and GTX 470 as a Gaming HTPC combo rig and it used almost 200watts at idle and yes it was always on.

The HTPC in my sig is almost done (Asus D1 already shipped) and I had a few of the components already. I just put in the GPU and TV tuners about 5-6hrs ago...

It idles around 100watts but I have to double check (killed my Kill-A-Watt) and doesnt use much more recording, I'm guessing no more that 130-150watts max, probably much less.

I know that isnt what you asked but it is something to think about it the meantime.

this one seems great!! in a review someone said that a 4890 was REALLY tight... Does the 460 take less place?

i agree it should be separate because you can get a very very small case for an HTPC but not a Gaming pc. I would do this
Case
Gpu
Core i7 2600K
Micro ATX mobo
4 GB ram
Blueray
Nice sound card
BIIIIG HDD
