It really depends on what your goals are. What language are you looking to use?

Notepad++...you need to know what you're doing as well as understand the proper syntaxing.

I've seen too many people use Dreamweaver and have simple issues because they don't understand what they're doing.

Dreamweaver, it helps to see the code formatted, also to see demos quickly of what you are coding.

This. The code produced by Dreamweaver, etc. is pretty ugly. I would recommend hand written code.

Photoshop for whipping up the graphics.
Then depends what language you have to use.
If I'm restricted to running on php then VS is out for obvious reasons.
If its classic ASP then Visual InterDev from VS 6
If its ASP.net (preferred) then VS.Net as I wouldn't even know where to begin with dreamweaver running and debugging ASP, does it do it?

As for hand written code in notepad etc. sounds hardcore and whilst OK for editing files, surely a little naive to do the entire web dev with it? Even the best programmers need to debug their code so why not use the tools that help with that (ie stepping)

From experience I can tell you that the workflow that arises from using Dreamweaver is far more convenient. Using photoshop to make your layout then exporting to fireworks and then to Dreamweaver to code allows great speed increases in menial tasks.
Visual studio is good, its just that it doesn't allow for a high speed organized project.

i do this which is great.

BUT EVERYONE IS WRONG

If you want to start off with really bad habits then pick one of those. Personally if you are a complete noob you need to use the basic notepad and get a book or go to w3 and practice, practice, practice. Once you feel ok with the HTML, then move to CSS. The reason why i say this is all other programs will spoil you. After you get the concept, understanding, and can read the language plus be able to say you can write it by hand then move to the other programs. I would suggest Notepad++ it is the best for code other than designing it in Photoshop and throwing it over to fireworks.
