Trouble with these cards and these errors, hard to pinpoint. I had the same issue, turned out to be a busted row of pins on the motherboard under the CPU.

Hope that isn't the case for you. Have you tried installing the 10.12 drivers? Just for reference.

Im currently in Safe mode cleaning the remaining ATI drivers out then ill try to install the 10.12 drivers and i will not install the CCC cos i heard that can cause some problems too.

I am having the same issue with my dual HD 6850's. They ran fine until I removed them. I have not tried to play a game yet, but when I open CCC it locks up. Maybe my issue was a result of possibly reinserting them in a different order after having the drivers installed; however, reinstalling the drivers did not help. I suppose I will try to switch them and see if the problem is resolved.

Also, have you had your motherboard looked at? Maybe something is messed up on the board itself like Arthur Hucksake stated.

As it turns out, one of my cards was simply defective out of the box. I guess returning it seems to be the best solution for this issue. Especially since each fix posted seems to only work on a case-by-case basis, unfortunately. I wish I could offer more support.
