How long have you had this card? Is it still under warranty? Is the CPU actually POSTing?

If the CPU is posting and you're getting black screened, sounds like it may be a bad card.

Well what it sounds like, is your GPU is dying/dead. I am a bit confused though. Are you actually turning the GPU off and then things run OK? Or are you changing settings and still using the GPU and things run OK?

Hmm so when you restart your CPU you don't have any problems? Sounds like a motherboard or driver problem then. But you've already cleared and reinstalled drivers. I'm not really sure what to say. Have you OC'd your CPU or GPU at all?

Do you have another video card that you can use to make sure it's your HD 4670 that's giving you problems?

Have you overclocked your CPU or GPU? And as Onoz said, do you have another GPU you can test with?

Edit:

When you have your card running 'stable' after restart, can you run a stress test like Furmark to make sure your GPU is stable?
