Just stick with high.
There isn't a huge noticeable difference anyways.

It probably IS your CPU. Your CPU is hitting 75%, and Modern Warfare 2 only uses 2 threads. 2 threads on a tri-core CPU = 75% total usage, meaning CPU bottleneck.

I played it on an old K8 Opteron dual core (Socket 939) clocked at 3.0GHz. His tri core isn't holding him back.

I'll bet its a SLI issue personally (assuming the card in his sig).

Alright, I need to ask a few questions here. I see your PSU is a 600 watt, how long have you been running those 2 9800 GX2's, your PSU could be straining to give power to all the parts. SO you now have this stuttering going on as a effect. How long have you had the PSU? It could have a decreased power output than when you first bought it. Take out a GX2, and see if that helps. If it doesn't then it might be something else. Edit: I seen people run MW2 on a 256mb graphics card, a heck of alot more than what is required.

Yeah his PSU produces less power than his computer even requires at full load. The OCZ ModXStream Pro 600W provides 504W total from the 12V rails. Two 9800GX2's alone need 394W, 95W for your tri-core, couple of hard drives are probably 12-13W, and few fans together are 10W+. Easily over 500W if you add all of that together. I doubt this issue is due to your PSU though, everything would have to be at 100% usage for you to hit that number. I'm not saying that running it that close is safe though.
I highly doubt you've seen it run at the highest texture detail with a 256MB vram card. OP if you want to dispell any thought of it being the CPU bring up something that shows CPU usage per core and play the game while watching it. If it maxes two cores and the third is left untouched it could be your CPU. If you can drop texture detail and it fixes the problem I'd be inclined to believe that it may be a lack of vram causing the problem. I'm also assuming that the SLI stuttering issues that I've read about in the past have been remedied? If not, that could be your problem right there.

I played MW2 on a AMD 939 at 2.6GHz with a 8800GS with 384mb VRAM on high settings with no problems. I doubt its the CPU.

I doubt its a CPU issue, i have same CPU and a single 9800GT with only 4gb ram and im not having any troubles...
I might say he Is having VRAM troubles depending on his resolution?
Like if he's trying to play at 1900x1200, very high settings with AA and stuff, then it could be choppy...
but with my system i run crysis and other games on high/vhigh with ease at 30 fps minimum, other games almost always more...
so i cant see the CPU being the problem?

Edit: Just read his monitor is a 19"er so probably not a resolution prob.... maybe PSU is what im thinking...

extra textures cause stuttering issues on some nvidia cards

was same way on my 9800GTX+
with high textures everything was fine though
