Im also reading on overclocking and what not.

Answer 1. Do you have cpuz? The way I do cpuz is press the button at the bottom and then at the top of the valoidation page put in my name and my email and press validate, give it a min or so and the web page pops up copy and paste the web address and there you go, it has nothing to do with dual monitors.
Answer 2. That is a pretty new bios and alot of members here have it, so it's up to you to update. How to update, your driver cd should have a program called Asus update and that is a program that should have been installed if all the other drivers and programs were installed, simple follow the on screen prompts and there you go.
Answer 3. reading first=good, but it looks like you have a descent setup, did it require that many volts or did you set it there saying" that should be enough"? cause that seems kinda high to me as I get 4.ghz with 1.3875 volts, but every chip and setup is different. What were your temps after P95ing for 12 hours?

OH and welcome to OCN

If you read through some of the post here in this section you'll see what me and some of the others have done. as far as those volts, Nforce HTv and NBv you should be able to leave them alone unless your overclocking using the cpu bus as for the VDDA set it to 2.8 to help with the vdroop as much as possible cpu/nbv you should set it around 1.3 to help the built in memory controler that is in the cpu and then you could possible set your cpu volts alittle lower not sure by how much. Overclocking to me is really just a trial and error kinda skill and alot of reading up on the basics of OCing. as far as temps those are really good temps in my book, as for the cpu the max temp is 62c, so your a good amount below it with 50 but I wouldnt want to push it any higher for 24/7 usage.

it could be your chip also ive got a x2 555 that hates anything over 4.0 regardless of volts or temps even on this board or my M4A88T-M, but my 955 can hit 4.0 without even trying?

I have done a lot of reading, and while this board can have a bad vdroop on some boards (like you know who's lol) the phenom II is really only meant to hit 4ghz ~ 4.3ghz stable unless you are using phase changing! I don't see one P II at 4.5ghz or 4.8 or 5ghz that is truly 100% stable but were this board truly falls a little (little) short is the cpu/nb feq. if the board is able to get to cpu/nb 3000mhz that is where the p II shines but 2800mhz is nothing t sneeze at! The AMD chip is broken up in to two different ways and the memory controller is the best way to see performance then the cpu feq. just stop trying to hit 5ghz and work on the nb feq. and you will see the big difference in your FPS and Benches
