Yes, more than likely the problem is related to the hdd.

Install problem hdd as a storage drive dl and run hdtune and wd diagnostic tools ( assuming wd ) and see if the hdd comes back with errors. If still under warrantee set up a rma with manufacture and get it replaced.

The raw read error rate has risen a bit, I think WD hard disks normally have that one at 0. That *might* be the cause of the freezing problems, but hard to know for sure. Otherwise everything is OK with those SMART values. Is the new (working) drive the same type (brand, model) as this one?

Did you run the quick error scan? Although it is quick, it often can't find errors even if there are some.

I've had situations where HD tune finds no bad sectors, but the HDD manufacturer's diagnostic tools do, so you should use WD Data Lifeguard to confirm the error scan results.

Those values seem pretty much perfect, no problems there.

If the freeze happens again, you could also check Windows Event Manager afterwards to see if any disk errors have been logged there. At least on XP, disk errors are found on the System tab of Event Viewer. Here's an example screenshot of disk errors:
