I have successfully OCed my i7 920 to 3.8GHz (24h prime95 stable, 15h Linx). Temps are OK, 83C max.
My settings:
200x19
vCore: 1.3v
VTT: 1.34v
CPU PLL: 1.82
HT: Yes
CStates, Throttling, Speedstepping: Yes. I wan't to keep these and HT on.
Everything else is at stock.
If I set it to 200x20 I can't even boot.
191x21 and vCore at 1.35v I can boot, but I get a BSOD after <1h of Linx.
I've tried playing with the other voltages (CPU PLL, IOH, ICH, etc) but I haven't been able to get it stable, and I haven't kept a proper log, so I'm ready to start from scratch.
Try these settings out. I'm assuming this is a D0, so you may need to make adjustments if it's a C0:
Ai Overclock Tuner : Manual
CPU Ratio Setting : 21
Intel SpeedStep Tech: Disabled
Intel Turbo Mode Tech isabled
Bclk Frequency: 191
PCIE Frequency: 101

DRAM Bus Voltage: 1.64v
You might also try 211x19 to get your RAM running at 1688Mhz.

The thing is I'd rather be at 3.8GHz w/ all that stuff than 4GHz without, so I'm shooting for getting it stable with all that, or not.
@everyone else: thank's for the replies. I'll reread (I had read it before, but forgot to come back to it) that thread, and try those settings. I'll leave it testing throughout the night, I'll report back tomorrow (unless I smell smoke or something, lol)
What I've figured out:
x20 multi sucks on my CPU: with 211x19 or 191x21 I can boot windows, but with 200x20 I just can't, no matter what settings.
IOH, ICH, CPU PLL, etc don't do anything for me: I tried them one by one, together, and in pairs, and none of them helped me.
1.34v VTT is the right point for 3.8GHz, and 1.41v for 4GHz (based on the BSOD meanings of the thread that Darren linked)
The stablest (1-4m Linx) I got was 191x21 @ 1.35< vcore (i.e., I tried up to 1.4v, and it didn't help), 1.34v VTT, 1.82v CPU PLL, evereything else at stock (voltage wise, I disabled spread spectrum, set QPI to the lowest multi, and adjusted RAM settings).
Even though I think it can be quite challenging overclocking with the power saving features enabled, I'm sure your goal can be achieved.
One thing that will help is to manually set as many options in the BIOS as you can. Especially, your RAM settings (DRAM Voltage: 1.600V, DRAM tCL: 8, DRAM tRCD: 8, DRAM tRP: 8, DRAM tRAS: 24, DRAM Command Rate: 2T).
Read through this guide, and see what progress you can make.
Then, come back and post your latest settings. If you encountered a BSOD during stress testing, post that error code, and I'll see if I can point you in the right direction.
Note: Try not to let your load temps get any higher.

typical for bloomfields that 20 multiplier is crazy.
my tip. loose timings on ram, or if oced, set to original clocks or as near as possible. or even try to lower them below the rated clocks for finding your way for 4.0 GHz. memory overclock got influence on cpu overcklock. try to push your pci-e clock for 101 or more.
check your vrm cooler. if you will find that all mosfets are imprinted on thermalpad its ok, but when one is not, you should be carefull and dont try to oc your cpu.
My latest settings:
Everything set manually to stock. IEST, C1E and C-States enabled.
VTT: 1.34.

@1.3625v vcore: temps get up to 88C, computer shut down after 5m of Linx.
@1.5v vcore: 3B BSOD
Now testing 1.5625 (I think it was) vcore.
I thought it was within a step of TRUE, Megahalems, etc?
My setup will surely make it to 4GHz in winter (now ambient is probably ~33C, in winter it's more like 15C, so my temps would go from 87C to 70C, i.e.: I could probably do 1.4v vcore)
My current vcore: no, that was surely a typo (2nd one voltage related on this forum, lol, they always cause confusion). I'm at 1.3625v according to my BIOS (if my memory serves me well), 1.344v according to CPUz and OCCT
Holy cow!! If you're correct in your CPU VCore, it's way too high. You could be close to frying your chip. You should keep it under 1.400V to be safe. Also, you really shouldn't need anything higher than 1.350V for 4 GHz.

Idle temps, and stress temps are way different my friend. And lower Your CPU vcore, You want to fry Your chip? 1.6 is way too high. And no, You couldn't do 1.4 with mugen lol. As far as I know Megahalem and True are better performers than mugen

To clarify:
I'M OBVIOUSLY NOT RUNNING 1.6v vCore. THAT WAS A TYPO!.
Right now I'm running 1.3625v vcore, and 30m into Linx with a max temp of 87C on one core (86C, 87C, 81C, and 82C for all four different cores).
I just got "CPU too hot!" (87C as per my OCCT settings) 29m into Linx (actually, I use OCCT Linpack, but It's ok if I call it Linx right? They'r both linpack based...) max settings
211x19 appears to be more stable on my board (i.e, with 1.325v I could boot into Windows vs not being able to with 191x21).
At 211x19 w/ 1.35v vCore, VTT=1.34v, CPU PLL=1.82v, 1.65v DRAM (mine is rated for 1600 @1.6, so since it's a few MHz over I gave it a little extra juice) and evereything else at stock, OCCT Linpack freezes in <20s. CPU temps get to 80C (and then it freezes). I tried lowering the vCore (1.325), but I got the 124 BSOD.
I'm assuming that TurboMode is currently disabled. Please verify. Also, have you tried enabling it and setting 191x20?

Yes, Turbo is disabled. On my MB, the only way to enable it is to set the multi to "Auto". I'm pretty sure I tried that (191x20 w/ Turbo), but I'll do so again.
If I go any higher with the vcore, my temps get to high (>85C in Linx). What should I raise IOH CSI to? Should I raise it on it's own (I've been trying just raising IOH, ICH and ICH CSI to 1.22 all together, but it seems to make no difference). Which of these voltages affect the CPU heat? I have pretty good airflow, so I could probably up the ones that don't quit a bit (if it helps).

If I lower the vCore I get the same thing (freeze) or if I go way lower (eg: 1.325) I get a BSOD.
I do game some, altho it's not all I do. I'm planning to sell this rig in <6 months, and go for a lower end overclockable sandy bridge CPU and a better GPU (the whole reason I went x58, i7 vs P55 i5 is because I wanted HT and such for encoding, but now a lower end CPU+quick sync does that for me, so I have no need for high end), so I need it to be rock stable and run at whatever speed I say it runs at even if it's the middle of summer.
Should I lower the vCore a notch at a time until I get a BSOD and then report it here?
Bah, I live in argentina man, I can't just buy a new cooler...
So I guess 4GHz isn't doable on my setup? Sucks my CPU needs >+.5v for those extra 200MHz
