What applications? What type of files would be accessed most common on the file servers? How complex are the websites?

ONE server for Active Directory? You're asking for trouble. Each of the 5 file/print servers should also be configured as Domain Controllers with Global Catalog, DNS and DHCP roles installed.

Key question - what's your budget?

No set budget. Just something logical
1 app server hosting medical manager software
1 app server hosting business manager software
The files will be the files created by these medical and business softwares
When I said active directory i meant a server that that provides all network services is that a bad idea too? you think i should have more than 1?
The website is pretty simple.
I was thinking i get a bunch of dell poweredge r710's and select the hardware appropriately for select needs. I'm just not sure what requires more processing power and ram.

A "bunch of R710's" is overkill for what you need done.

A single, properly configured R710 can function as a virtualization host and run several of those servers on a single physical box.

Microsoft Active Directory is, well, a directory service. And yes, it's a bad idea to only have a single directory server. I'd suggest you become familiar with AD, Global Catalogs, DNS, DHCP and AD-Replication as a starting point.

In this case, I would get a single R510 server loaded with HDDs and configured as a virtualization host. This would run the 4 servers you listed, excluding the 5 branch office servers.

At the branch offices, a single R310 with redundant power supplies would likely be enough, again, depending on how many users at each location, and only if you have existing rack infrastructure to use. Otherwise, I'd get a T410 tower server configuration with 4x HDDs and redundant PSU. The HDDs would be configured in RAID-10 (or RAID-5) configuration to provide improved file access to users.

Alternatively, each branch office server can also be configured as a virtualization host. This gives you the option of segregating a domain controller from a file server.
