MAG has 256 players working just fine with modes similar to conquest or rush.
You just need to design maps with the intention of said size in mind.

For example, have multiple spawn locations other than just two or three that move as one team progresses.
Also make sure that places where objectives are located have multiple methods of getting in, out, and defending. Not just one or two methods of getting there. Strike at Karkand for example, one of its initial flags has only two or 3 ways of getting into it. Stairs or jump from the rooftop. With 256 people, that doesn't work at all. You need to be able to approach said objective from 4 or more directions and to be able to defend the objective too from the same amount. It needs balance.

Even not just one or two objectives either, make it possible for 4 objectives at once or more to be accomplished. In MAG, in order to progress from one stage of combat to the next, there is usually like 4+ locations to be captured. And in order to capture the locations fast, you need alot of people present at the one location. It takes a long time to capture it otherwise, and there is always waves of people coming due to 256.

Anything can be designed right with that amount of players or even more. It just needs to be tooled appropriately and designed with it in mind.
You can't place 256 players in a map designed for 60 or less and has only 2 objectives with very few ways of getting in and out of them. Obviously it won't work.
If you have a building with 256 people in them that's designed for only 60 people and it has only 2-3 exits, and you start a fire, xD is all I gotta say.

256 players is an excellent way to demolish the competition (CoD)... With large maps, jets and other vehicles, 128 players per side is easily doable and can be balanced. The firefights will be incredible and awesome and DICE should definitely do it. However, they should do it like this:

Have different servers with max number of players (64-player servers, 128 player servers, and 256 player servers).

Those with only the most advanced and powerful computers/connections can play the large servers (kick based on ping and frame rate(s)) and that way, we will get balanced games with varied sizes. And, this offers another advantage. If someone is bored of playing on a 256 player server, they can go to a 64 player server and do 'smaller' maps and blast. If one wants huge maps and large teams, go to the 128/256 player servers and have fun. Ultimately, the choice should be left to the player with some constraints added in to maintain smooth playability for all.

It would be a travesty if they have the technology to do 128 & 256 player servers, and I certainly believe DICE does indeed have that technology, and not implement it for some trivial reason(s).

The issue is, myself and many others find 64 player too large. Quite often, I loved playing 12 vs 12 on BF2. Also, to put 256 players on a single map, it has to be massive... but you also have to limit vehicles. So, what happens to those on the 256 players maps that have no vehicles!?

I just want 64. I would love to see more but for me it's like moving from an F430 to an F458... sure it's an improvement but I would be incredibly happy with either.

40-64 is the perfect range on large maps for me, 24-32 on small maps. At some point, you either get chaos, or you're so far away from the majority of the players that it almost makes ^no difference.
