PC screen messing up, HELP! (GPU issue)?

So now I'm extremely annoyed I recently had to RMA my PC back to cyberpower as it would not boot, before I did this I installed a Zalman VF3000a to my 5870, worked great that day then the PC wouldn't boot I have finally recieved my PC back from cyberpower after waiting a month for them to install a motherboard and "test" the pc. I booted it this afternoon and everything was fine played some music and web browsing and it worked fine, I then played crysis 2 and shut down the pc and windows began some updates. I get back in The evening and boot and it appears the text on the bootup screen began jumping everywhere. After I got into windows I mangaged to play music for about 2 minutes and then parts of the screen began to disappear, at which point the screen resets to being a black and White vertical lines and I am forced to manual reset, with te same issue.I'm beginning to think this is a graphics card issue but I'm not certain. I checked inside the case, everything seemed fine except I noticed my exhaust fan was unplugged so I plugged it back in.

On booting it decided not to boot so I did startup repair and system restore same issue.

Also not sure if this is relevant but I noticed (only during boot process) that the mouse and keyboard seemed dead however when I clicked the mouse and it's LED appeared, same with keyboard. So maybe it's a power issue.

TL;DR. After boot into windows my pc screen goes blotchy and reverts to a black and White vertical line screen, so I have to restart.

Stupid question:

You installed the latest drivers?

I have, although not sure if any have been released in the last month? Also I'm not sure I'd have time to check if I booted now, as it messes up pretty darn fast.

have you tried a differnt port on the graphics card? however, it seems to me that the card itself is screwed up, perhaps from the zalman cooler install? did you forget to put TIM on the gpu?

I have put thermal paste their, however some of the VRM or RAM are not covered with heatsinks on the Graphics card as it is not reference. I read a review on benchmark reviews and the reviewer had the same model
GPU as me Powercolor PCS + 5870, and he had to buy spare heatsinks, I could buy these but I'm not sure it will fix anything?

Also it seemed to work fine originally when I installed the cooler.
