Evga 560ti problems/artifacting

So, my 560ti seems to be dying or it's already dead.. I've been using it for the past 2 weeks and last week it started doing some crazy stuff while playing crysis 1.. now it's happening when I play almost all games, I took 10 pictures of artifacts while playing bfbc2. All happening on stock clocks! Please check the pics below:

I overclocked it, but I haven't gone too far, it shouldn't be doing this, guess I got a crappy one.. the max overclock I've ever done was 915/1830/2150 (1.012v), the temps never gone above 75ºC.. I already tested right after clearing cmos, with everything on stock settings and it doesn't stop artifacting.. also formated windows, installed all drivers, removed all cables and plugged it back again, it doesn't stop no matter what I do.

I'm almost entirely sure I screwed my gpu and guess it's RMA time, but I have a few questions:

1 - Could it be due to another piece of hardware? like my PSU (I know it sucks) or the cpu overclock itself, North Bridge or RAM memory?

2 - Since it's an evga brand, guess I won't have any problems with warranty due to overclock, right?..

All in all, I would strongly recommend you guys to be really carefull while overclocking these cards, I haven't gone too far and it's screwed as you may see..

I don't know whats causing it but my PS3 does that white spots thing around objects, so annoying, makes my eyes hurt.

Sounds like you were simply unlucky. If it's doing that at stock clocks, go for an RMA. The 560s generally OC pretty well; I ran my first at 970/2300 completely stable (stock clocks now that I have two), and I believe 950 core is achievable for most cards, even on reference cooling like EVGA's.

Not enough voltage, maybe? I'd give it another boost, maybe +.02 and see if it clears up.
