i7 like odd multipliers. They also like their highest possible odd multiplier. Depends on the chip.

Try 23 x 174 (175 if you got stuck at 3999.98MHz like I did ) @ 1.28v. Up it in small increments after BSoD like usual.

This guide is a great starting point...Using the reccomended settings word for word. I am running my sig rig using 1.25 for 4ghz... http://www.clunk.org.uk/forums/overclocking/22106-core-i7-overclocking-guide-beginners.html

I used 175 x 23 to get 4.025GHz. This was the original OC. Didn't get past 4GHz in CPU-z validation. Tried 175 and got 4.025GHz.

Here are my settings for a i7 950 D0 4.2GHz with HT off.

I have had a i7 920 D0 previously and found it very similar with my 950 D0 with the 950 needing approx 0.0125 less voltage to do same speeds as my i7 920 D0.

You need to look for a code like 0x000000124, or 0x000000101. Those are QPI and Vcore, respectively, btw.
