Several parts of Call of Cthulhu: Dark Corners of the Earth qualify...especially that damn hotel scene where you wake up...and I don't spoiler if I can avoid it.
When the face hugger first jumped on my face, and its tentacle thing underneath is all squishing up against the screen, I literally jumped back and fell of my chair.
The part in F.E.A.R. when you're crawling through a vent and your flashlight flickers then dies out. Then it comes back to life, just in time to show Alma crawling at an abnormally fast pace at you.
I yelled, swore, and pushed myself away from the desk so hard that my headphones came flying off. In that order.
When I played Minesweeper for the first time. I was just clicking around, then BAM! out of nowhere I hit a mine. I flipped over backwards in my chair, spilling coke and chips all over my pants area.
