Doesn't really matter. There are only a few rads that I know of that include both. One being the swiftech MCR220 QP and some magicool radiators.
There will be no temp difference in going from a MCR220 with integrated reservoir to an MCR220 with a separate reservoir.

Not quite sure what you're asking here. Some people want everything to be internal instead of having a bunch of stuff outside the case.

I prefer to have the reservoir (when I use one) separate from other components as I like to shake the radiator and turn it upside-down when bleeding, I also like to turn components at angles to help get all the air out.
Having a res on the radiator might hinder this... but I am just guessing.

I would say have the radiator separate. As convenient as it might seem to have them together, apart is probably better for troubleshooting if you do have an issue, as well as when you drain your WC setup.
