Mine does that sometimes. This Vista 64 is very unstable. Nvidia drivers for Vista64 suck.

I can just about guaranty that your using either the wrong drivers (32 bit probably) or 32 bit programs that should be 64 bit.
Fully update it as well. Re-formatting is a bad idea when you have no clue what an issue is. Also, your CPU is 24 hr. Orthos stable, and memory is 10 pass memtest86 stable right?

Spot on there mate

Lower your overclock if necessary and run memtest86+ to ensure that your RAM is stable. Bearing in mind that the Superfetch version of Vista makes more robust use of your RAM than XP, it's likely that your current clocks aren't as stable as you thought they were.

i reformatted to x64 just yesterday and its fantastic. Finally im using my cpu to its full potential. I would say a driver issue? maybe a codec you installed?
Control Panel usually lists third-party codecs under their own section which may be the issue when it tries loading the codec not realising its incompatible. Check Windows "problems and solutions" if theres anything there that hasnt been resolved.. usually driver issues... then solve it. The Intel ICH9 chipset for example has an issue with vista if you install the motherboard cd drivers. you have to download the latest intel driver to solve that issue.
