At the moment, Attachments are uploaded separately and differently from how it used to be. Understandable since you can now upload ZIP files, but it adds some confusion. My suggestion is to edit the IMG BBCode button to perform extra functionality to consolidate all pic related functions, closer to how OCN worked previously.

At the moment, you need to upload an image as an attachment, then click attachments again (which results in a different behavior than at first), and select an attachment to inline. Like so;

Current IMG button behavior is to just prompt for a URL in a pop-up window;

I suggest adding a similar drop down to the IMG button that already exists on the attachment button;

The idea is to add the following functionality;

Press the IMG button and instead of getting the popup, get a drop down menu similar to the attachment button that has options for;

Add from URL (same as current IMG functionality, popup asking for URL)
Upload from file (same as adding attachment, but also auto insert image)
Add from attachments (same as current attachment dropdown functionality)

So maybe look like this;
