Make sure that HyperTransport speeds are on stock.
Touching this screams instability.
The next thing is CPU-NB, make sure you are using stock clocks there as well.
If you want to OC the CPU-NB, make sure you use at least 1.4V for it.

Test the GPU with something like FurMark.

Test the RAM with Memtest86, just so you know its not something by bad oc on the CPU-NB.
