Your PC crashes, not the DP or the HDMI port. Your problems can be caused by a software error (reinstall is required), RAM settings (speed/timings, overclock (set to stock), CPU overheating (get a different cooler/clean heatsink, ), VRM overheating (cool them/ensure airflow through the case), GPU/VRAM overheating (ensure proper airflow, clean heatsink, lower speeds), PSU being weak (replace it).
You haven't told us what your hardware is so we can't help you but I've given you a check list for you to look at.
