Im not 100% sure, but some people are reporting that they are not seeing the banner at the top anymore, so I believe it may be dead.
Just follow the on screen instructions, it's really simple.

About the fee
If you read the page before you keep clicking next, it says the fee is waived, and they will send you a verification code in your email.
