UPDATE: Hi all,

I have removed the form for now. Well over a hundred of you submitted feedback, and we thank you sincerely for it given that it was not really a short form!

I will spend the next couple days finishing up my review/summary of everyone's comments, and will also prepare a report to update you all on what everybody else was saying.
