Raise your QPI/DRAM voltage to 1.3v or higher if needed (max is 1.35v). You may even be able to lower your CPU voltages with a higher QPI/DRAM voltage.
21x195 is not 4.0, but yeah I agree with extreme that you should try raising vtt could be ioh as well. I'd raise your qpi/uncore to around 1.25 just to not have to worry about any kind of droop.
i would raise your Qpi slightly before you try increasing your vcore, if that doesnt work, then increase your vcore by 1 increment seeing as how high it already is. what are your load and idle temps btw?
I found that QPI/DRAM voltage was the key when overclocking my i7. Before raising the vcore you should try 1.35V for your QPI/DRAM and see if it becomes stable. If it is you can lower this voltage until it becomes unstable, then bump it back up to your last stable setting.
