More voltage isn't always the answer.
You need these chips to stay COOL to get more from it. Remember the more voltage you throw at it, the hotter it's gonna run.
If you'll take a look, some of these chips have broken 4.0 with voltages under 1.50v's and I can tell you Phenom II chips hate the heat. I've noted the Phenom II chips once they get to around 50C or so flake out in a hurry and most if not all will do it at this temp. Be sure the rest of your system is good temp-wise too since every component in the system matters and it's only as good as the weakest link present.
All it takes is your MB or someting else getting hot/wonky/
/ (whatever) to wreck an attempt for more MHz.
I was getting around 4.1GHz max on air from mine with good ambients and after I set it up with H2O, got the speed shown in the spreadsheet. I don't know exactly how you have your watercooling setup but there are things you could try.
