MAJOR PROBLEMS! need help

Ok so im trying to OC my 9850BE with a M3A78-T motherboard.. and i have been trying many different settings on just the CPU ALONE. the last 1 i tried which I thought would be successful.. wasnt! im completely lost at what i could possibly be doing wrong.. It was set at 14.5x with a 1.375v and ACC was all cores +2% and when I ran Prime95 i ran it on Small with the FFT from 8-32 and 1 min each stage.. well it got to the last stage and gave me a blue error message from windows about damaging my hardware.. and it has done this on the last few that i have tried.. but this was the longest i have gotten it to keep testing at this lvl.. which the OC was 2.9ghz and the temp even at 100% for around 12mins solid never got over 55c so i have no clue what im doing wrong.. I have posted a few times but im still lost on it.. any and all assistance would be very helpful! thanks guys!

I would recommend trying ACC at a higher setting. 4/4/4/4 then 6/6/6/6.
Try throwing a few more volts at the CPU if you're encoutnering errors. In the case of AMD systems, it's also wise to make sure you aren't overstressing the HT link too much.
stock voltage can reach 2.8Ghz stable if thats the case 1.375v should be 3.1Ghz or 3.2Ghz..try doing the stock voltage and set multiplier to 14..run prime and see if anything goes then try 1.37v with 15 multi..as you can slowly but surely reach 3.2 or 3.1 stable..
