or could it be my hardware?
please guys help me out..
  1. Have you tried reinstalling your ATI drivers?
  2. Made sure your DVI/HDMI/VGA cables are securely in place and screwed tight?
  3. Tried the monitor on a different machine?

^^ this. usually if there are lines in the screen especially black ones the CFC cables are torn or messed up. this is just from my personal experience. usually this dosent happen unless you open the monitor and flex the cables, but who knows.
My vid card has 2 video outs, I have 2 monitors. Recently my 2nd monitor starting acting weirs, lots of artifacting and pixelization. I switched the cables to the other video out, and it went away.

the monitor is borked. rma time. if it does it with different sources like you described.
you aren't using an hdmi switch or nothing to test, right? connect directly
then like other person said, if it's doing it on pc and ps3, i'd say it's the monitor

That is not a 'native resolution', per say...

Is the monitor 1920x1080? And are you running at that? The whole '1080p' thing is just a marketing gimmick. (strong opinion of some people, including myself) It is not a 1920x1200? Just double checking! :)
