OCZ Vertex 3 MAX IOPS Help.

I have a OCZ Vertex 3 MAX IOPS SSD that is acting wierd.
Most of the time from a cold boot it isn't there or if you let it off for about 3-4 minutes. But if you reset the rig and it is there again. It was working fine in another rig and testing it out side the rig, but this happens when I build a friend rig with it in there. Same mobo/ram/PSU/HDD and I added a Blu-ray/3 HDD too it.
Is it a sign that the SSD is failing?

Secure erase it and reinstall?

Update it's firmware?

Update the mobo's BIOS version?

System overclocked? That can cause issues for boot.

Normal format and install.
Mobo is up to date.
Firmware idk.
It is at stock.
clear mobo cmos yet and reinput your settings?

see if you can update the drive's firmware
Well I finially just use a WD VelociRaptor for my friend main HDD, but the OCZ SSD works fine in my system so far without disappearing.
