USB Memory Stick says Please insert disk

I have a 16GB USB storage bar and when i put in it says "Please insert disk into removable disk"
Is there any way to get this thing working again?
I have tried a Format and it wont do it!
And i have tried it in other PCs

I've tried it in three PCs, 2 running Vista and the other running xp

I also tried "File Scavenger" but nothing

Try formatting it using HP USB Disk Storage Format Tool.
