I dont know quite what category I come under as far as mice go.
As a gamer of a senior age, and who suffers from Arthritus in my hands and wrist, I find that my ancient Microsoft Optical trackball mouse is best for me.
I can use just my fingertips on it or for games that need less sensitive movements then resting the heel of my hand on the mouse is fine.

Too bad it only had 3 buttons though. That rules it out completely for me.

I haven't read the whole thread but agree with people saying the FK1 is perfect for people with small/thick hands, and close to ideal for people with medium & large hands.

I disagree with some of OP's points.

1. You CAN make a mouse that's ideal for all 3 grips for the vast majority of people (IE: a wider FK1).

2. Thinness isn't a requirement for a fingertip grip mouse. I felt like the FK1 was uncomfortably thin even in fingertip grip. Though I do have large hands.

Any mouse that's shaped so you can grip the mouse firmly with your thumb and ring-finger and pinky. Usually means a | or ) shape for your thumb and a | or very shallow ( shape for your ring/pinky. Basically the only mouse you can't fingertip is /_\ and \_/ shaped mice one example is the old razer diamondbacks they had this really steep \_/ shape to them which made getting a firm grip impossible.

To me the best mouse ever was the G9/G9x wide grip shape. The mouse was was and low profile(large humps get in the way of the fingers) hell the mouse was as long as my middle finger but it had the right shape to it that allowed a firm grip on the mouse.
