I would check temps and then check the drivers to see if they are fully updated. It may also be your psu and I've never heard of that brand so idk how good it is compared to rosewill/corsair or something like that.
 
Reinstall video card drivers.
Try another video card (if possible).
Check the HDMI cord to see if it has a broken pin or something.
Try a DVI-VGA if none of the above work.

EDIT: Lol I used to have almost all the same parts in your rig except the 480.

Different PCI port maybe? Is your card sagging at all?

HDMI cords are a huge problem right now because some are made cheaply and cause a ton of problems so I would check that and make sure the cable is securely in the slot.

The monitor should be fine as long as its cool enough that it won't burn your hand when you touch it.

The card may sag a little but I doubt the 480 would have too much problem. I would check to see if the card is sitting correctly in the slot because if it doesn't get good contact it may cause those problems.
